EATING OUT ON A DIET

Eating at a restaurant isn't license to blow your calorie budget. Staying within your caloric means while dining out -- and enjoying your dining experience -- comes down to making a wise restaurant choice followed by wise menu choices.
Since you haven't let yourself get overly hungry, you can carefully evaluate the restaurant choices available. If possible, choose a restaurant that offers a wide variety and prepares food to order -- it will be easier to find healthy options.
Whether you end up at a fast-food joint in town, a food court in a mall, or in a neighborhood of fine restaurants, a wise restaurant choice will give you the chance to have better menu options.

Restaurant Tips

The following are some general guidelines for staying on your calorie budget:
  • Take your time surveying the selections. Look for "heart-healthy" or "light" selections. Or, if there's a senior section on the menu, opt for the smaller portions offered there.
  • Be assertive. Ask your server questions to clarify how things are prepared. You're the customer, so don't be shy about making special requests or substitutions -- just keep them reasonable. For instance, ask for fruit or tomato slices instead of hash browns at breakfast; any restaurant should have those options on hand. Or ask for a menu item to be baked or grilled instead of breaded and fried.
  • Order first so you're not influenced by others in your party.
  • Consider ordering a la carte. It may cost more, but it can sometimes give you better food options.
  • Drink a glass of water before your meal arrives.
  • Choose an appetizer as a main dish and accompany it with a salad or broth-based soup (not a cream-based soup) that's full of beans or vegetables, such as minestrone or gazpacho. Make sure the appetizer is not deep-fried.
  • If others are eating appetizers before the meal, order a small salad or a cup of broth-based soup.
  • Ask the server to remove foods that are tempting, such as a basket of chips or bread and butter.
  • Choose small or medium portions. A 16-ounce steak is enough meat to give four people a 4-ounce serving (which cooks down to the recommended 3-ounces). And that 1-pound steak can pack about 1,000 calories, whereas the 4-ounce piece will have only about 250.
  • Share an entree.
  • Ask for the vegetable of the day to replace higher-calorie items that come with your meal. Have the vegetable steamed or otherwise prepared without fat.
  • Don't join the clean plate club. It's all right to leave food on your plate. No one likes to be wasteful, but it's better to leave it on your plate than put it on your waistline.
  • Take half home. To avoid temptation, ask your server to put half your meal into a box before bringing it to the table so you won't eat more than you plan. (However, if you won't have access to refrigeration within two hours, leave the leftovers behind.)
  • Be picky about splurges. Enjoy foods you don't normally eat at home, and save the regular "treat" items such as potato chips for another time, since you can have those anywhere.
No matter what kind of restaurant you go to or what the cuisine, there will be higher- and lower-calorie choices on the menu. And there will be menu choices that better fill your daily nutritional needs as well as choices that are primarily empty calories. The trick is knowing what to look for on the menu. The next section covers the decisions you should be making when faced with certain food choices.

Why US Adults Use Dietary Supplements

Background Dietary supplements are used by more than half of adults, although to our knowledge, the reasons motivating use have not been previously examined in US adults using nationally representative data. The purpose of this analysis was to examine motivations for dietary supplement use, characterize the types of products used for the most commonly reported motivations, and to examine the role of physicians and health care practitioners in guiding choices about dietary supplements.
Methods Data from adults (≥20 years; n = 11 956) were examined in the 2007-2010 National Health and Nutrition Examination Survey, a nationally representative, cross-sectional, population-based survey.
Results The most commonly reported reasons for using supplements were to “improve” (45%) or “maintain” (33%) overall health. Women used calcium products for “bone health” (36%), whereas men were more likely to report supplement use for “heart health or to lower cholesterol” (18%). Older adults (≥60 years) were more likely than younger individuals to report motivations related to site-specific reasons like heart, bone and joint, and eye health. Only 23% of products were used based on recommendations of a health care provider. Multivitamin-mineral products were the most frequently reported type of supplement taken, followed by calcium and ω-3 or fish oil supplements. Supplement users are more likely to report very good or excellent health, have health insurance, use alcohol moderately, eschew cigarette smoking, and exercise more frequently than nonusers.
Conclusions Supplement users reported motivations related to overall health more commonly than for supplementing nutrients from food intakes. Use of supplements was related to more favorable health and lifestyle choices. Less than a quarter of supplements used by adults were recommended by a physician or health care provider.

DHEA for weight loss: miracle drug or unproven experiment?

In a study exploring DHEA-replacement therapy as a weight loss technique, researchers gave test animals large doses of both DHEA and the drug fenfluramine. According to Dr. Ray Sahelian's DHEA: A Practical Guide, "Even a hot fudge sundae with fresh bananas and dark chocolate syrup couldn't get [the test animals] interested." If you're like most sweet-toothed adults, you probably can't imagine not being interested in a hot fudge sundae. You're probably thinking, "There's something that can make me not interested in a hot fudge sundae? Give me some of that!" But if your taste buds haven't entirely taken over your mind, you might also be asking whether animals would normally be interested in a hot fudge sundae. Unlike the researchers, you might conclude that the animals' lack of interest in ice cream doesn't necessarily prove there are weight-loss properties in DHEA and fenfluramine.
If you have come to the conclusion above, you have also inadvertently realized the problem that is inherent in most DHEA-replacement therapy research. Scientists performed most of this research on animals, and what works for animals may not work for people. This leads to an obvious discrepancy between results from animal-based and human-based DHEA studies. As Dr. Sahelian writes, "[A study] done at the University of Rochester in New York evaluated the administration of 1,600 milligrams of DHEA for four weeks to eight healthy men. The researchers did not find that this steroid had much of an influence on weight loss or energy and protein metabolism. Epidemiological studies also do not support the theory, as had been proposed based on animal studies, that high serum DHEA(S) levels protected against obesity and diabetes."
It is important to note that even human DHEA studies frequently contradict each other. In PDR for Nutritional Supplements, Sheldon Saul Hendle and David Rorvik wrote, "A report in 1988 that high-dose DHEA could favorably affect lipids and induce weight loss in young males was not confirmed in two subsequent trials. In another trial, using the same 1,600-milligrams daily dosage of DHEA used in the 1988 study but this time in women, there was, again, no weight loss."
Contrary to the findings of the University of Rochester-based study, however, in a Temple University-based study, DHEA-supplements enabled adult men to lose 31 percent of mean body fat, as cited in Joseph B. Marion's Anti-Aging Manual. Furthermore, Marion writes that a Temple University researcher, "…reported that DHEA can help a person lose weight by blocking an enzyme known to produce fat tissue." Although the results of human-based and animal-based studies differ for obvious reasons, how can these equally reputable, human-based studies be so contradictory?
Perhaps the discrepancy can be attributed to the questions posed by the researchers, rather than the resulting answers. Researchers who specifically try to label DHEA as a diet pill will be unable to do so. Contrary to what the pharmaceutical companies formulating DHEA supplements as a prescription drug would like you to believe, DHEA supplements are not diet pills. DHEA is not a "quick-fix" cure to obesity.
In order for you to grasp how DHEA supplements sometimes promote weight loss, you must first understand the role of DHEA in the human body. DHEA, or Dehydroepiandrosterone, is an important steroid hormone made in the adrenal glands from cholesterol. DHEA levels decline with age. As Marion's Anti-Aging Manual details, "DHEA levels are known to fall precipitously with age, falling 90 percent from ages 20 to 90. DHEA is like the hub of a wheel and is the central hormone that is a precursor to the numerous sex hormones (including estrogen and testosterone)." In other words, if your DHEA levels are falling -- if the hub of your hormone balance is breaking -- then your body will feel and show the effects. The symptoms that postmenopausal women know all too well, for example, result from these decreased levels of hormones, including that of DHEA.
If you know anything about middle-age weight gain, it should come to no surprise that "body mass index (BMI), a measurement of obesity, was higher in those with low DHEA-S levels," according to Life Extension Foundation's Disease Prevention and Treatment. With this in mind similarly, the fact that DHEA sometimes aids in weight loss should also come as no surprise.
If you're wondering why DHEA isn't 100 percent effective in combating weight loss, ask yourself this: Are any young adults, who presumably have high DHEA levels, obese? The answer is: Yes, of course. Obviously, there are multiple paths to obesity, and DHEA supplementation only helps reverse obesity when that obesity is caused by a DHEA deficiency -- which rarely occurs in the young.
At any age, however, you can still lose body fat safely and naturally, without the use of DHEA supplements. Pursuing a healthy lifestyle, including following a healthy diet and exercising regularly, is a sure way to lose body fat. It's not a magic pill, but it works.

The experts speak on DHEA and weight loss

Note: in the citations below, you will find conflicting statements on DHEA and weight loss. This is no mistake. Even in the natural health community, there are conflicting opinions on the efficacy of DHEA for weight loss.
A landmark study of 242 men, 50-79 years old, based on 12 years of research, stated that a small supplementation of DHEA (100 mcg/ml) corresponded to a 48% reduction in death from heart disease and a 36% reduction in death from any cause (other than accidents). A Temple University medical researcher reported that DHEA can help a person lose weight by blocking an enzyme known to produce fat tissue. In one study, DHEA supplementation enabled men to lose 31% of mean body fat with no change in body weight in 28 days. Another study, involving 16 middle-aged to elderly men, showed that taking DHEA for one year led to a 75% increase in their sense of well-being—they coped better with stress, felt more physically mobile, and slept better.

At this dosage there was a lowering of cholesterol and a decrease in body fat, with a greater response in obese individuals. Most DHEA supplements on the market are less than 50 mg.

These researchers recruited 13 men and 17 women who ranged from 40 to 70 years of age. Using a randomized, placebo-controlled, cross-over trial, they provided 50 mg of DHEA nightly for 3 months. During the study period they measured blood levels of many hormones and nutrients including androgens, lipids, and insulin, as well as body fat, libido, and sense of well-being. Other researchers went on to find that DHEA was involved in converting the body's excess glucose to energy— without DHEA, the glucose turned to fat. Too much fat can lead to obesity, and obesity can be a key factor in premature death. Recently, DHEA has been proven to play a role in protecting the thymus gland so that it can do its job in fighting disease. Put them all together—protection against obesity, infectious disease, and many forms of cancer (in laboratory animals)—and you have a hormone and a gland that are obviously very important in the ongoing internal battle against aging. Significantly, levels of both melatonin and DHEA decline markedly with age.

DHEA, growth hormone, and testosterone are hormones that can restore aged muscles to a youthful anabolic state. In fact, growth hormone does something no other weight-loss regimen does: It recontours the body, melting away fat and building muscle. In many cases, people look like they've shed years away along with the fat they have lost. Even better, the greatest loss occurs in deep belly fat—the area associated with increased risk of heart attack. In every study of growth hormone's effects on "normal" people who are aging, GH reduced body fat and increased lean body mass. What we do know is that DHEA has a significant anabolic effect, which results in stronger bones and muscles and decreased body fat. It may protect health in a variety of ways. This study compared DHEA-S levels in the blood of 423 people who practiced Transcendental Meditation as compared to 1,253 healthy people who did not. The ages ranged from 20 to 81 years. Depending on the age range, people who practiced Transcendental Meditation had levels of DHEA-S that were as high as members of the control group who were five to ten years younger.

I’m sure you heard all about the new research on the Mediterranean Diet that made headlines worldwide.  This large-scale study from Spain, published in the New England Journal of Medicine, got tons of attention because it was the first time we’ve had such strong evidence supporting the benefits of this style of eating.  Participants following the eating patterns common in Spain, and other coastal countries surrounding the Mediterranean Sea, had a significant reduction (about 30%) in the risk of stroke and other cardiovascular events.

No doubt the results were solid — and the researchers even suspended the study early (after five years) because it was so clear that those eating the Mediterranean Diet were benefiting in such significant ways that the control group wasn’t — there were still critics of the study.

Some experts claim the design was flawed because the control group did not follow a low-fat diet – and others thought the media over-hyped the results.  Despite the media brouhaha, and the study’s potential shortcomings, there are worse things than drawing attention to a diet rich in fruits and vegetables, whole grains, seafood and healthy fats like extra-virgin olive oil and nuts.  Plus, did I mention the wine?  Those following the Mediterranean Diet enjoyed a glass of red wine with meals.  Maybe we can’t conclude that the Mediterranean Diet is more beneficial than a low-fat diet, it’s certainly a better approach than the way many people eat in this country.   It’s also a style of eating that celebrates food, encouraging the pleasures of the table without a long list of restrictions.  I say that’s all good.

Maybe the Mediterranean Diet got a lot of praise in the press.  Perhaps the study’s results were over-hyped.  I’m OK with that. I would much rather have people eat like a Spaniard, Italian, Greek or Lebanese than eat like a Caveman.  It’s great that this style of eating got the type of attention that’s usually reserved for the latest fad diet.  All too often — the science-based, sensible approach doesn’t make news, or doesn’t sell books.  I hope this will help change that.  Two fantastic dietitian colleagues Meri Raffetto and Wendy Jo Peterson just wrote a book, the Mediterranean Diet Cookbook for Dummies, which can show you how easy and delicious it  can be to eat like you’re from the Mediterranean.

We certainly enjoy Mediterranean-style meals at home. Here are some ways you can bring a bit of the Mediterranean to you every day.

Do not fear the fat. It’s still hard for some people to get over the idea that not all fats are bad. The Mediterranean diet is rich in healthy fats, such as extra-virgin olive oil, nuts and avocados.  Make your own vinaigrettes with olive oil instead of using bottled dressings.  It will be fresher and tastier – and you’ll even help enhance the absorption of some nutrients from the salad compared to using a low-fat dressing.  Another recent study found that the aroma of olive oil helped to enhance satiety, or feelings of fullness.  Saute your vegetables in olive oil, snack on nuts instead of chips or pretzels, and find new ways to enjoy avocados beyond guacamole.

Make vegetables a center-of-plate star. Forget the notion of starting your meal planning with a big slab of meat.  Find ways to have vegetables play a bigger role. As Americans, we often start our meal with a single salad drenched in a creamy dressing.  It’s much different in Lebanon, where we visit my in-laws every summer.  The Lebanese enjoy an array of vegetables during a meal, including salads made with some of the most nutritious greens you can eat –   tabbouleh with parsley, fattoush with chopped vegetables, sauteed chickory with caramelized onions, and fresh rocca salad with beets.  Meats are often served as kebabs, so you can enjoy a small portion after eating the vegetable-packed mezze.  Find ways you can shrink your meat portions — enjoy pastas that are studded with vegetables and just  a little meat, make a hearty stew with beans and small chunks of meat, and experiment with whole-grain entree salads topped with sliced roasted chicken.

Love your lemons. I always keep a big bag of fresh lemons in my fridge that I use to squeeze on vegetables or make salad dressings during the week. There’s nothing quite like a squirt of citrus to brighten up a dish.  On the weekends, I will often squeeze a bunch of lemons and make a batch of dressing with olive oil, grated fresh garlic and kosher salt. I keep this elixir in a bottle in the refrigerator and use it on everything – roasted cauliflower, arugula salad, sautéed broccoli and pasta dishes.

Face your fish-cooking fears. Dietary guidelines recommend 8 ounces of fish a week, but most Americans fall short of this goal.  For many people, it comes down to not knowing how to prepare fish and seafood at home.  It’s not a difficult task.  You’ll find lots of ways to gain seafood-cooking confidence in my new book, The Food Lover’s Healthy Habits Cookbook.  Eating seafood twice a week is one of the 12 healthy habits featured in the book. Embrace beans. Warning for athletes: Some dietary supplements may contain trace amounts of banned substances

(NaturalNews) Athletes should be cautious about the dietary supplements they take, as these may contain small amounts of banned substances, according to a recent study conducted by Loughborough University Professor of Sport and Exercise Nutrition, Ron Maughan.

Having repeatedly warned about the possibility of a link between failed doping tests and sports supplements, Dr. Maughan believes that these commercially available substances can turn honest athletes into unknowing cheats. "It is now well established that many dietary supplements contain compounds that can cause an athlete to fail a doping test. In some cases the presence of these compounds is not declared on the product label," the professor explained. The extent to which minute amounts of substances that are forbidden in the sports world should be relayed on the product package is still under debate. Some voices believe that trace amounts are not relevant enough to be mentioned, and, most of the time, they do not turn up on regular supplement analyses.

However, Professor Maughan is worried about the presence in everyday supplements of some steroid compounds normally banned by the World Anti-Doping Agency (WADA). Such a steroid is nandrolone, and Dr. Maughan put together a team to investigate the body's responses to trace amounts of a nandrolone precursor (19-norandrostenedione). The test subjects involved were given water and protein bars that had been contaminated with 19-norandrostenedione at a concentration level 1000 times lower than what is normally tested for during the manufacturing process of supplements.

Surprisingly, even at this apparently insignificant concentration, the tested athletes still registered a positive result in common doping tests. This means that the sports dietary supplement industry's standards are much less strict than the health standards of the sports community. This incongruity can only have a negative impact on individual athletes who had regularly used supplements until this day.

In addition, various recent nutrition studies have revealed that the body should derive all of its nutrient requirements from natural food sources. As such, the scientific community is generally in agreement that supplements should be avoided if possible, in favor of a healthy and balanced diet that includes many raw vegetables and fruits.

Professor Maughan believes that his findings expose "a serious dilemma for sports supplement manufacturers, athletes, and those responsible for the welfare of athletes." He also added that "The potential for such low levels of contamination in a sports supplement to result in adverse test results raises significant concerns for the manufacture of dietary supplements intended for consumption by athletes liable to regular doping tests."

Professor Maughan currently presides over the Sports Nutrition Group of the International Olympic Committee Medical Commission, and has expressed further interest in the science of anti-doping. He is presenting at two upcoming conferences on this matter.

Digestive Enzymes: Help or Hype?

As interest in food intolerance grows, supplemental digestive enzymes are gaining traction with consumers. Increasingly, when patients show me their multivitamin labels, there are a handful of digestive enzymes listed among the vitamin and mineral ingredients. I've also read online testimonials from folks who swear that enzymes marketed as providing "digestive support" for gluten work as promised—and enable them to eat bread and pasta, bloat-free.
What are these enzymes, exactly? Should you be taking them?
Enzymes are proteins that facilitate specific chemical reactions. Digestive enzymes facilitate the chemical breakdown of food into smaller, absorbable components. Enzymes called amylases break down starches into sugar molecules; proteases break down proteins into amino acids; and lipases break down fat into its component parts.
Humans naturally produce multiple different enzymes in these families that encounter food at different places in the digestive process: first in the mouth, then in the stomach, and finally, within the small intestine. Humans also possess disaccharidases, or enzymes that break the bonds between double sugar molecules like sucrose (table sugar) and lactose (milk sugar) into two individual sugar molecules for absorption.
Unlike members of other biological kingdoms, humans (as mammals) lack digestive enzymes that break down compounds from plant cell walls like cellulose, pectins and resistant starches. These dietary compounds are collectively called "fiber." By definition, fiber is a nondigestible substance whose health benefits derive from its inability to be absorbed. Since it survives the digestive process intact and unabsorbed, it can travel along to the colon where it feeds our resident bacteria and contributes to fecal bulk.

In considering whether taking supplemental digestive enzymes may be beneficial, one should recognize that the term "digestive enzymes" is a catchall that includes a variety of compounds with different purposes—similar to "vitamins" or "probiotics." Just as we can't draw sweeping generalizations about whether taking vitamins is beneficial (it would depend on which vitamin in which individual), so too with digestive enzymes: It depends on which enzyme and in what population.
There are a handful of circumstances in which supplemental digestive enzymes offer a very clear benefit, so let's start there.
• Pancreatic insufficiency (PI): This condition, in which the pancreas is unable to produce or excrete normal amounts of digestive enzymes into the intestine, is treated with enzymes taken orally with meals so that food can be absorbed.
Unlike over-the-counter enzymes, these prescription enzymes are specially coated to resist being digested by stomach acid before they can do their work in the intestines. Signs and symptoms of PI include light-colored, foul smelling, loose stools—often that float as the result of malabsorbed fat.

For example, many people with cystic fibrosis require supplementation with enzymes since pancreatic insufficiency is a hallmark characteristic of the disease. People with chronic pancreatitis or who have had pancreatic surgery may require supplemental enzymes as well.

• Lactose intolerance: The lactase enzyme is required to break down and absorb milk sugar. In many people, the body ramps down its production of lactase in their teens and twenties. With low levels of lactase available, large quantities of milk or other dairy products cannot be comfortably digested.
For dairy-loving folks with lactose intolerance, lactase enzyme supplements are widely available and effective—if taken at a sufficient dose along with dairy foods. (Alas, it may require some trial and error to titrate the right dose based on the product you're using and the portion of dairy you're eating.)
• High-fiber diets. People following high-fiber diets, and often vegetarians and vegans, who rely heavily on beans and large portions of veggies as a cornerstone of their diets, sometimes find that a little bit of enzymatic help from a friend in the fungi kingdom can make a huge difference in their diet tolerance.
Enter Beano—a popular digestive enzyme product that contains alpha-galactosidase, a mold-derived enzyme that's required to break down the type of fiber found in beans and gassy veggies like cabbage, broccoli and cauliflower. Taking multiple Beano tablets with a plant-based meal that's high on the flatulence meter can help minimize the gas that follows.

Beyond these circumstances, the benefits of supplemental digestive enzymes become murkier.
The notion that large swaths of the population suffer from "enzyme deficiencies" is advanced by some in the health arena—and particularly so by profit-oriented marketers of digestive enzyme supplements. In fact, apart from lactase, overt digestive enzyme deficiencies are rare, and they generally occur in malnourished, ill individuals—not in gassy but otherwise well people.
It's also worth noting that many labs performing stool analyses of pancreatic enzyme levels use a different reference range than that which is conventionally accepted as normal. These alternative standards tend to result in findings of "pancreatic insufficiency" at a much lower threshold. Lastly, some degree of intestinal gas is perfectly normal; its presence does not necessarily imply that something is amiss.

Most gastroenterologists dismiss over-the-counter digestive enzymes as a placebo, at best. "You'll digest them well before they'll be able to help you digest anything," says my colleague, New York-based gastroenterologist Eric Goldstein, regarding plant-based enzyme products that lack enteric coating, which enables intestinal absorption. He notes that these products are substantially different from FDA-regulated, prescription enzymes. His advice? Once all medical reasons for your symptoms have been ruled out, try chewing your food better, get breath-tested for suspected food intolerances, take Beano if it helps and avoid foods that bother you.

Still, some practitioners contend that over-the-counter digestive enzymes have benefited their patients with digestive woes, even in the absence of an overt enzyme deficiency. Integrative nutritionist Liz Lipski and author of the book, "Digestive Wellness," is one such clinician. She acknowledges that scientific evidence supporting the benefits of digestive enzymes is lacking but recommends these products based on her practical experience that they work in certain cases.
"If someone tells me they consistently feel bloated or gassy two to four hours after eating, or that food feels like it just 'sits there' hours after a meal, I've found that support from digestive enzymes in these cases can make a real difference." Lipski, who is based in Laurel, Md., is careful to note that she never uses enzymes as a first-line treatment for such digestive symptoms, as it's important to first rule out causes that would require conventional dietary or medical treatment—like celiac disease or Small Intestinal Bacterial Overgrowth (SIBO).
The bottom line: There are easily two dozen reasons why one may feel abnormally bloated or gassy—they range from serious medical conditions to completely benign side effects of a healthy, plant-based diet.
